Job Summary: This position forms part of The Wellness Center (TWC)
Care Coordination Team, comprised of 3-6 Wellness Navigators overseen by a Care
Coordination Manager. The goal of the team is to help clients pursue their
health and wellness goals by engaging clients and ensuring access to services
and resources. Wellness Navigators
facilitate for clients the communication and coordination of services offered
by the 16 partner organizations housed at TWC, services offered by these
organizations offsite, services offered by the LAC+USC Medical Center, other
public agencies and community organizations.
In addition to staffing the Information Desk at The Wellness Center, the
Wellness Navigators may also be actively engaged in outreach in the community and with clinical
providers on the LAC+USC Medical Campus (outpatient clinics, urgent care,
emergency room and hospitals).
Duties and Responsibilities:

Works with the 16
TWC partner organizations to create and modify a master schedule of activities
and services provided by partner organizations.

Develops relationships with public agencies
and community based organizations to build knowledge of the resources available
to clients.
·
Engages in community outreach to reach the
target population.
·
Engages with
clients referred to TWC by clinical providers in the LAC+USC clinics and other
community clinics via the electronic referral system or other referral
mechanisms.
·
Completes client
intake/registration and referrals.
·
Identifies
services/resources that help patients meet their health and wellness goals.
·
Makes appropriate
referrals and coordinates finding/scheduling services for clients.
·
Completes
appropriate documentation to track client use of TWC services.
·
Provides accurate
and timely feedback to referring clinical care teams when necessary.
·
Provides culturally and linguistically
appropriate information to clients.
·
Maintains a professional disposition while
working with clients, TWC partner organizations, and the multidisciplinary
healthcare team.
Minimum Qualifications:
*4 year college degree that includes
experience/training in social work, health education, public health, health
systems, pre-clinical professions training, or related fields
*2 years of experience in working with medically
underserved communities
Necessary Knowledge, Skills and Abilities:

Bilingual
(English/Spanish)
·
Great
organizational, interpersonal, oral and written communication skills
·
Office skills:
email, internet navigation, Microsoft Office products, electronic scheduling,
electronic referrals, answering telephones, voicemail, copy/scan/fax
Desirable Qualifications:
·
Familiarity with public agencies and
community based organizations in/near Boyle Heights
·
Familiarity with
Los Angeles County agencies, including DHS,
DMH, DPSS, DCFS & DCSS
