Job Summary: This position forms part of The Wellness Center (TWC) Care Coordination Team, comprised of 3-6 Wellness Navigators overseen by a Care Coordination Manager. The goal of the team is to help clients pursue their health and wellness goals by engaging clients and ensuring access to services and resources.  Wellness Navigators facilitate for clients the communication and coordination of services offered by the 16 partner organizations housed at TWC, services offered by these organizations offsite, services offered by the LAC+USC Medical Center, other public agencies and community organizations.  In addition to staffing the Information Desk at The Wellness Center, the Wellness Navigators may also be actively engaged in outreach in the community and with clinical providers on the LAC+USC Medical Campus (outpatient clinics, urgent care, emergency room and hospitals).

Duties and Responsibilities:
·         Works with the 16 TWC partner organizations to create and modify a master schedule of activities and services provided by partner organizations.
·         Develops relationships with public agencies and community based organizations to build knowledge of the resources available to clients.
·         Engages in community outreach to reach the target population.
·         Engages with clients referred to TWC by clinical providers in the LAC+USC clinics and other community clinics via the electronic referral system or other referral mechanisms.
·         Completes client intake/registration and referrals.
·         Identifies services/resources that help patients meet their health and wellness goals.
·         Makes appropriate referrals and coordinates finding/scheduling services for clients.
·         Completes appropriate documentation to track client use of TWC services.
·         Provides accurate and timely feedback to referring clinical care teams when necessary.
·         Provides culturally and linguistically appropriate information to clients.
·         Maintains a professional disposition while working with clients, TWC partner organizations, and the multidisciplinary healthcare team.
Minimum Qualifications: 
*4 year college degree that includes experience/training in social work, health education, public health, health systems, pre-clinical professions training, or related fields
*2 years of experience in working with medically underserved communities

Necessary Knowledge, Skills and Abilities:
·         Bilingual (English/Spanish)
·         Great organizational, interpersonal, oral and written communication skills
·         Office skills: email, internet navigation, Microsoft Office products, electronic scheduling, electronic referrals, answering telephones, voicemail, copy/scan/fax

Desirable Qualifications:
·         Familiarity with public agencies and community based organizations in/near Boyle Heights
·         Familiarity with Los Angeles County agencies, including  DHS, DMH, DPSS, DCFS & DCSS

Volunteer Opportunity

Attention MPH students!

Get Covered Illinois is looking for volunteers to work directly with Medicaid applicants that qualify to fill out and submit their applications and other important activities related to engaging the community to promote a culture of coverage like working with currently trained Navigators/Certified Application Counselors. You will be trained on how to answer basic questions and make sure consumers are being appropriately directed to complete applications for coverage.
Location details are being finalized for these events and are planned to take place throughout the city and suburbs at hospitals beginning March 29 and ending on March 31. All you need to do to participate is to fill out this doodle: https://doodle.com/nx868gip3b9ffmkr

Congratulations Prof. Emily Benfer, Recipient of the 2014 Schweitzer Leadership Award!

On behalf of the Public Health Policy and Management faculty and students, I extend our heartfelt congratulations to Professor Emily Benfer, the recipient of the 2014 Schweitzer Leadership Award! This prestigious honor is bestowed on an individual who has done significant work to mitigate the social determinants of health in the community, and whose commitment to service has influenced and inspired others.

As the founder and Director of the Health Justice Project, a medical-legal partnership at Loyola University Chicago School of Law, Prof. Benfer has fought tirelessly for the rights and well-being of our most vulnerable populations, including homeless families, children with special health care needs, individuals with disabilities, and low-income workers.

While her accomplishments as a Director of one of the most successful medical-legal partnerships in the nation will be recognized many times over in the weeks ahead, what is (in my humble estimation) worthy of equal praise is her exemplary character and leadership. I have had the privilege of working with Emily as a researcher in her Health Justice Project, an educator co-teaching our University’s first interprofessional course, and as a colleague on a practice commission. These experiences have enhanced the prestige of our University, promoted interprofessional education across multiple disciplines—medical, law, and public health—and created research and service opportunities for our faculty and students. But simply put, they are a product of her leadership and vision.

It is a rare individual who brings the passion, professionalism, productivity, and personality that are hallmarks of a great leader. And this year’s recipient of the Schweizer Leadership Award exhibits all of these traits, and is an inspiration to all of us who are lifelong learners and practitioners of health policy and management. Congratulations, Emily!
