Quality Improvement Facilitator Internship

Position Code 13-1382
Location: Oakbrook, IL

Position Summary:
The purpose of this 12 month internship is to coordinate and implement quality improvement initiatives with Community Care Transitions Programs. This may involve researching data, developing project plans, monitoring quality improvement activities, individual case or record review, serving as a resource and providing education to team members, providers, practitioners.

Essential Functions:
Facilitates health care activities, project participation or review and implementation with physicians, providers and team members, ensuring plans, tasks and timelines meet the Company and contracting agency requirements.

Develops and maintains activity and/or project plans. Ensures plans, tasks, and timelines meet contractual and Company requirements and guidelines. Provides regular updates to Company management on activity and/or project status.

Researches and gathers technical and clinical data to support activities and/or project initiatives. Performs analysis on the results of the data collection. Based on the outcome of the analysis, determines areas for improvement. Provides recommendations feedback to project participants. Assists in the development of project materials and intervention tools.

Provides education on improvement methods and processes to providers and practitioners. Assists in the implementation of the methods and processes.
Performs miscellaneous duties as assigned.

Requirements:
Currently enrolled in a Master's degree in Public Health and valid RN, LPN, RHIA or RHIT and/or 3 – 5 years experience in Wellness, preventive health, health care quality improvement and/or medical; experience in specific areas as needed/designated by the contract or project; functional PC knowledge; significant experience in focus area of project(s).

Positions working on specific contracts may require U.S. Citizenship.
Because of the nature and immediacy of the work, the ability to maintain regular and predictable attendance is essential.

Additional Comments:
60% local or overnight travel.
Experience with quality improvement initiatives preferred.
Experience with community care transitions preferred.
Bilingual (Spanish/English) strongly preferred.
